• Application form, properly filled in with capital letters.
• Two photographs.
• A medical certificate, allowing the practise of fighting sports, issued recently (not over 3 months old).
• Copy of an identity card.
• 31,00 € payed with check or post order, to the order of the F.I.B.D/D.A

If you have already got your licence, and you would like to extend its validity, please forward the following documents :

• Application form, properly filled in with capital letters.
• One photograph.
• A medical certificate, allowing the practise of fighting sports, issued recently (not over 3 months old).
• 25,00 € payed with check or post order, to the order of the F.I.B.D/D.A.

To become a F.I.B.D/D.A instructor, your F.I.B.D/D.A licence must be up to date (see here how to get the first licence or renew your licence). Moreover, you are kindly required to forward the following documents :

• Application form, properly filled in with capital letters.
• Four photographs.
• First Aid Certificate.
• A medical certificate, allowing the teaching of fighting sports, issued recently (not older than 3 months)

To affiliate a club to the F.I.B.D/D.A for the sports season, please forward the following documents :

• First affiliation form or affiliation renewal form for this sports season.
• Acceptance slip for the club's executive commitee.
• Affiliation settlement (100,00 € for the sports season 2017-2018).
• Settlement for the executive commitee's licences (minimum 4 licences, 25,00 € each licence for the sports season 2017-2018)
